MSHSL Form (see below) – Print out the Athletic Eligibility Brochure and signature form, sign both sides and bring into Athletic Office.
Physical Exam Forms – Use this 3-page form when athlete gets an updated physical. Bring completed form into the Athletic Office.
Arlington Athletics Registration Forms - Print out the 4-page form which includes Code of Conduct, Information Sheet, Equipment-Uniform-Refund-Locks-Locker Form, and Website Authorization Form. Bring completed forms into the Athletic Office. Link below....
RETURNING ATHLETES FROM A FALL SPORT
Print and complete the Returning Athlete Form at bottom of page and bring it into the Athletic Office with your fees.
